Who Can Apply? 

• Lung transplant recipients (both pre-transplant and post-transplant) 
• Primary caregivers of those on a lung transplant journey 

Eligibility Requirements 

• You must be a Saskatchewan resident 
• Your expenses must have been paid within the past 12 months 
• Your application and all supporting documentation must be completed in full 

What Expenses May be Reimbursed? 

• Medical expenses
• Travel and accommodations 
• Fees and other eligible expenses

How Do I Apply? 

Your request won’t be reviewed until we receive all required documents. 

Step 1: Complete the online application: Lung Transplant Financial Assistance Application Form

Step 2: Upload your receipts when completing your online application. We do not accept quotes or estimates. 

Step 3: Submit a Health Care Provider Referral Form.

You can submit the referral form in either of these ways: 

• Upload it with your online application, or 
• Your provider can email it directly to: caringbreaths@lungsask.ca

After careful review, applications are prioritized based on medical and financial need and are subject to available funding.  Please allow up to 90 days for the review process to be completed. You will be contacted by email once a decision has been made. Any reimbursements will be provided via cheque and mailed to the mailing address provided in the application form.
